Notification of Medical Condition
An email was sent home this week with form 316-1, Notification of Medical Condition & Personal Care Request/Authorization. This form only needs to be completed and returned to the school if your child has a medical condition the school should be aware of or medication which needs to be administered at school.
Students with severe allergies should have already been contacted to complete or confirm form 317-1, Severe Allergy Alert Form, and therefore form 316-1 is not required. If your child has a severe allergy and you have not been contacted, please contact the office ASAP.
If you have any questions or would like a paper copy of the form, please contact the office. If the form is required, once completed you can send it back to school with your child or email a copy to Mrs. Hayhurst.
